Regan MacNeil, una niña de doce años, comienza a experimentar cambios inquietantes en su comportamiento. Lo que al principio parece una alteración emocional o un problema médico pronto se convierte en una sucesión de fenómenos inexplicables que aterrorizan a su madre, Chris. Desesperada y sin encontrar respuestas, Chris recurre a la Iglesia, donde el padre Damien Karras, un sacerdote atormentado por sus propias dudas, y el experimentado padre Merrin se enfrentan a una presencia demoníaca que parece haber tomado posesión de la niña.
